Congratulations! You just completed the Ottawa County Business Advisory Council’s CHOOSE Program. We hope that you gained experience and have an understanding of what it is you want to do after high school.

In order to receive your school credit and stipend, please complete the CHOOSE Submission Form below:

  1. Student Timesheet: Download the Student Timesheet (click here) for your site supervisor to complete. You and the site supervisor will sign and date at the bottom. Once completed, please upload the Timesheet to the CHOOSE Submission Form.
  2. Student Presentation: Submit your experience and what you learned in the form of a Power Point Presentation. Be sure to include at least seven (7) photos in your presentation (we want to see what you’ve been doing). Include answers to each of the following questions in your Power Point:
    1. Describe 3 aspects of the job that you enjoyed or found interesting (3-5 sentences)
    2. Describe 3 aspects of the job that you found challenging or you did not enjoy (3-5 sentences).
    3. Would you consider making this position/job your career? Why/Why Not? (3-5 sentences) Once completed, please upload the presentation (PPT) to the CHOOSE Submission Form.
  3. OhioMeansJobs Readiness Seal: Download the OhioMeansJobs Readiness Seal document (click here). Complete the document, demonstrating 13 of the 15 OhioMeansJobs Readiness Seal Professional Skills. These skills can be demonstrated at different businesses. Once completed, please upload the OhioMeansJobs Readiness Seal to the CHOOSE Submission Form.

The CHOOSE Timesheet, Power Point presentation and OhioMeansJobs Readiness Seal are due May 1 of the current year. Stipends will be issued to students no later than May 15 of the current year.

Your parent/guardian will receive a copy of your submission.
Your School Counselor/Career Readiness Coordinator will receive a copy of your submission.
