CEO Internship Student Application Form
High School Junior and Senior students have an opportunity to earn high school credit for participating in the Ottawa County Business Advisory Council's CEO Internship Program. The CEO Internship program provides students with a pathway between school and work. Students develop entry level skills in a specific occupation, practice career management, and prepare for employment all while earning high school credit. The CEO Internship has several benefits:

-students can earn 1/2 credit and a stipend of $250 for every 60 hours of internship time completed
-the internship is flexible. This presents a wide-range of possibilities for hours/schedules (i.e. – can be done afterschool, during school, weekends, etc)
-the internship gives the students an opportunity to “test the waters” of a desired occupation

Student Information

Your parent/guardian will receive a copy of your application.
Your School Counselor/Career Readiness Coordinator will receive a copy of your application.
If you would like to intern for an employer over the summer, this would fall under the upcoming school year. (i.e. an internship beginning May 26 through September 12 of 2021 would be counted towards the 2021/2022 school year.)
Please list the days and hours/timeframes you are available. (i.e. I'm available Sundays through Tuesdays from 10am to 1pm, and on Saturdays from 1pm-8pm.)

The CEO Program is designed to give students an opportunity to explore a career in their area of interest. In order to participate, you, the student, must agree to and adhere to the following:

Disclosure

I fully understand the above statements and I agree to follow these requirements. If I do not comply, I understand that my school may choose to not allow me to continue my internship experience. I have completed the Student Information, the Student Internship Questions and the Student Agreement sections of the CEO Student Application Form.

Email Confirmation

Once you submit the completed form, you will receive an email confirmation along with a copy of your application (be sure to check your junk/spam folder).

The Student Internship Questionnaire is to be completed at the end of the internship by the student.

The Student Internship Weekly Log will be completed throughout the internship by the student. The student and the employer will need to sign and date.

Students are responsible for having their school and the employer complete the CEO School Form and CEO Employer Form.
